Introduction:

Responsibilities:
You will be responsible to:

prevent medication errors and enhance patient safety through effective systems and practices.
-
develop and implement policies and procedures related to medication use.
-
conduct regular audits and reviews of medication practices.
-
provide education and training to staff on safe medication practices.
-
conduct Failure Mode and Effects Analysis (FMEA) for high-risk processes, medications, and pharmacist interventions.
-
compile Key Performance Indicators (KPIs) and present findings where applicable.
-
design and implement technological solutions and system upgrades (e.g., smart pump libraries).
-
introduce workflow changes to minimize medication errors.
-
disseminate medication safety alerts.
-
conduct training sessions on safe medication practices.
-
ensure compliance with JCIA (Joint Commission International Accreditation), ASHP Hospital Pharmacy Accreditation, DRAP (Drug Regulatory Authority of Pakistan), Healthcare Commission standards, SafeCare International
-
represent Pharmacy Services in various quality forums within AKUH.
Requirements:
You should have:

PharmD (Doctor of Pharmacy) – required
-
Master’s degree in Health, Quality, or a related field – preferred
-
Certification in CPHQ (Certified Professional in Healthcare Quality) – preferred
-
Medication Safety Certificate (ASHP).
-
Minimum of 6-8 years of progressive professional and supervisory experience in a medium to large hospital including 3 years in a managerial capacity a tertiary care academic institution
