Responsibilities:

You will be responsible to:

• prevent medication errors and enhance patient safety through effective systems and practices.

• develop and implement policies and procedures related to medication use.

• conduct regular audits and reviews of medication practices.

• provide education and training to staff on safe medication practices.

• conduct Failure Mode and Effects Analysis (FMEA) for high-risk processes, medications, and pharmacist interventions.

• compile Key Performance Indicators (KPIs) and present findings where applicable.

• design and implement technological solutions and system upgrades (e.g., smart pump libraries).

• introduce workflow changes to minimize medication errors.

• disseminate medication safety alerts.

• conduct training sessions on safe medication practices.

• ensure compliance with JCIA (Joint Commission International Accreditation), ASHP Hospital Pharmacy Accreditation, DRAP (Drug Regulatory Authority of Pakistan), Healthcare Commission standards, SafeCare International

• represent Pharmacy Services in various quality forums within AKUH.

Requirements:

You should have:

• PharmD (Doctor of Pharmacy) – required

• Master’s degree in Health, Quality, or a related field – preferred

• Certification in CPHQ (Certified Professional in Healthcare Quality) – preferred

• Medication Safety Certificate (ASHP).

• Minimum of 6-8 years of progressive professional and supervisory experience in a medium to large hospital including 3 years in a managerial capacity a tertiary care academic institution
